Injury Treatment Massage

Auto Accidents / PIP, L & I, and payments at a later date are billed at $50 per 15 minute unit

Prior authorization and medical referral is required

Payment may be made prior to, or at time of service.

Cash payment is preferred.

Credit card and checks are accepted.

HSA and FSA cards are accepted.

A receipt can be provided for client to use for insurance reimbursement. It is the responsibility of each client to check with their insurance provider that all criteria has been met for reimbursement.

At least 24 hours advance notice is required for cancellation. Cancellations made within 12 hours of the scheduled service time will be billed full price.

For cancellations made between 12 and 24 hours before the appointment, the larger of $50 or 50%  of price of service will be billed.

Substituting a friend, family member, colleague, etc. to take the appointment will waive this fee. The fee will also be waived if appointment time is filled.

No Show: Not showing up to a scheduled appointment will result in the full amount due immediately. Online scheduling ability will be suspended until payment received and pre-payment may be required for future scheduling.

Under special circumstances these fees may be waived.
